How to Enroll

After learning about your coverage options and making your decisions, it's time to enroll.

Generally, any 2017 elections you make will remain in effect through Dec. 31, 2017. When your enrollment period ends, you may only make a change to your benefit elections during the year if you experience a qualified life event and report it through MyHR within 30 days.

New Hire Enrollment

Deadline to enroll 8 calendar days from date of hire 30 days of your life event
Benefits coverage you will receive if you do not enroll

Health: Employee-only ORANGE Plan option at non-discounted rate

Basic Life Insurance, Basic AD&D, Short-Term Disability, Long-Term Disability and Business Travel Accident Insurance, EAP

If you do not change your benefit elections on MyHR within 30 calendar days of a qualified life event, no changes will take effect.

How to enroll

Visit EPIC before the deadline

Visit MyHR through Connect within 30 calendar days of your life event.

When you will receive your enrollment confirmation statement

After your enrollment period ends, you will receive an email that your Confirmation Statement is ready for review in EPIC.

To review your Confirmation Statement, visit EPIC > New Hire > New Hire Checklist > Benefits Enrollment Process > View Benefits Confirmation Sheet. You can print or save this statement for your personal records.

Make sure you carefully review your Confirmation Statement. If you find an error, you should notify HR Services immediately at 866.324.6513 and within 4 days from the end of the enrollment period.

After you enroll, you will receive an email confirming that your life event has been processed. If you haven’t received confirmation then call HR Services prior to the 30 day deadline at 866.324.6513. To review that your coverage has been updated correctly, visit EPIC > Benefits Information > Current Benefits.

If you find an error, you should notify HR Services immediately at 866.324.6513.

The elections you make or default coverage you receive will remain in effect through the end of the year and you only will be permitted to make changes to these elections during the plan year if you experience a qualified life event and report it through MyHR within 30 calendar days of the life event effective date.

Note: Health Savings Account contributions can be changed at any time on MyHR.

ID Cards

If you enroll in the Health Plan, you and your covered family members will receive an Anthem BlueCross BlueShield (medical) and CVS Caremark (prescription) card in the mail within 30 days from the date your Confirmation Statement is available in EPIC. You also should receive a Health Savings Account (HSA) debit card from HealthEquity if you enroll in either the GREEN or ORANGE Plan option.

If you need to visit your doctor or use your pharmacy benefits before you receive your new ID cards, you can print temporary ID cards.

To print your Anthem BlueCross BlueShield temporary ID card, first call 855.285.4212 to request your ID number. Then, using your ID number, visit Anthem.com to sign up and follow the steps to print off a temporary ID card.

If you need a new or replacement CVS Caremark ID card, you can contact CVS Caremark at 866.587.4799 or visit caremark.com.

If you enroll in the Dental Plan, you will not receive a card in the mail. You can access and print your ID card online by logging in to Online Advantage at assurantemployeebenefits.com.
