COVERING DEPENDENTS

You must provide documentation verifying the eligibility of any new dependents you want to cover under your Kindred benefits. In order to ensure your newly enrolled dependent's coverage is reflected in the carrier's system by January 1, 2017, please submit the required documentation by November 22, 2016. However, you must fax or email the required documentation no later than December 5, 2016. The form is located in the Resource Center at www.kindredbenefits.com.
